Description
A fetus of approximately 23 weeks of gestation lies curled within the uterine cavity, seen at close range against the reddish, vascular endometrial wall. The head fills the lower field, showing fine scalp hair, a well-formed auricle with developing helix and antihelix, and fused, closed eyelids over the orbit. The trunk is flexed with the thorax and abdomen turned toward the viewer, the umbilicus marking the insertion of the umbilical cord, whose spiralled umbilical arteries and single umbilical vein are colour-differentiated as they course toward the placental surface. The upper limb is adducted with the hand near the face, while the lower limbs are drawn up in the characteristic flexed fetal attitude, the knee, foot and separated toes visible in the upper part of the frame. The twenty-third week sits at the threshold of viability, making this stage central to counselling on extreme preterm birth, surfactant maturation and neonatal intensive care planning. Biometric assessment at this point relies on biparietal diameter, head and abdominal circumference and femur length, while the anatomy scan window has closed only recently; skin remains thin and translucent over subcutaneous vessels, and vernix and lanugo are developing. The cord anatomy shown here is directly relevant to Doppler evaluation of umbilical artery flow, detection of a two-vessel cord, and recognition of nuchal or body cord loops. The flexed posture and fetal movement patterns are also used clinically as markers of neurological well-being.
