Description
At approximately 23 weeks of gestation the fetus lies in a flexed, curled attitude within the amniotic sac, viewed from the left side. The large rounded cranium dominates the body proportions, with the auricle of the external ear clearly modelled, the eyelids still fused, and one hand raised to the face in front of the mouth and nose. The lower limbs are drawn up against the abdomen with knees flexed and the feet crossed, while the umbilical cord emerges from the ventral abdominal wall, its coiled umbilical arteries and single umbilical vein visible through the translucent Wharton's jelly. Behind and around the fetus the amnion, clear amniotic fluid, the vascularised placental disc and the surrounding uterine wall define the intrauterine space. The second half of the second trimester is a clinically pivotal window: 23 weeks lies at the threshold of viability, and gestational age assessment, estimated fetal weight and amniotic fluid volume all guide counselling at this stage. This is the period of the detailed anomaly and growth ultrasound, when biometry, limb and facial profile views, cord vessel number and placental site and grade are systematically documented. The flexed posture, cord coiling and fluid volume shown here are also the anatomical basis for discussions of fetal movement patterns, oligohydramnios and polyhydramnios, and cord entanglement.
