Description
A fetus at approximately 28 weeks of gestation is shown in close view within the uterine cavity, lying with the head flexed toward the chest and the limbs drawn into the characteristic flexed fetal attitude. The face fills the lower portion of the frame, with fused-then-reopened eyelids now closed in rest, a formed nasal tip and philtrum, full lips, and the auricle of the ear visible in profile against the smooth, well-perfused skin. Above the head lie the shoulder, thigh and knee of the same fetus, folded so that the flexed lower limb crowds the space in front of the face. The umbilical cord passes horizontally across the field, its helical twist revealing the paired umbilical arteries spiralling around the single, larger umbilical vein. By the third trimester the subcutaneous fat layer is increasing, the skin is losing its translucency, and the fetus is capable of coordinated movement, sleep-wake cycling and response to sound and light. Cord position across the face and neck is a routine observation on ultrasound and colour Doppler at this stage, where nuchal loops and the number of cord vessels are documented; a two-artery cord prompts assessment for associated anomalies. Umbilical arterial Doppler indices at around 28 weeks are central to surveillance of fetal growth restriction and placental insufficiency, and the crowded intrauterine space depicted here explains the reduced but stronger fetal movements mothers report late in pregnancy.
