Description
A fetus at approximately 28 weeks of gestation is shown in tight flexion within the uterine cavity, viewed from close range so that the face fills the lower half of the frame. The eyelids are closed but distinctly formed, with lashes and defined orbital ridges, and the nose, philtrum and full lips are well modelled; the external ear shows a folded helix and antihelix typical of this stage. The umbilical cord passes obliquely across the frame just above the forehead, its helical twist revealing the single large umbilical vein spiralling with the paired umbilical arteries. A small fetal hand with separated fingers lies against the cheek, while the flexed knee and thigh occupy the upper field, and the smooth uterine wall forms the warm, diffusely lit background. By week 28 the fetus enters the third trimester, a threshold associated with markedly improved extrauterine viability as surfactant production rises, subcutaneous fat accumulates and the eyelids reopen after their mid-gestation fusion. The relationship of cord to face and neck depicted here is the anatomy behind sonographic assessment of nuchal and body cord loops, and cord coiling index is itself a marker studied in relation to fetal growth and intrapartum heart rate patterns.
