Description
A fetus of approximately 23 weeks gestation lies curled in the amniotic cavity, seen from the left side with the head directed inferiorly and the trunk flexed. The large, relatively broad cranium dominates the proportions typical of the second trimester, with the ear auricle modelled on the side of the head and the chin drawn toward the chest. Both upper limbs are flexed across the thorax so that the hands lie near the face, while the lower limbs are drawn up with the knees flexed and one foot presenting its plantar surface and individual toes. The umbilical cord emerges from the abdominal wall and passes upward toward the placenta, whose villous surface and vascular plate are visible against the myometrium of the uterine wall, and the smooth amnion encloses the fetus in fluid. By the middle of the second trimester the fetus has attained the threshold of extrauterine viability, making this stage central to counselling on periviable birth, surfactant maturity and neonatal intensive care planning. The posture depicted reflects normal physiological flexion and active fetal movement, features assessed during obstetric ultrasound alongside biometry such as biparietal diameter and femur length. Cord insertion and placental position, both shown here, are routinely documented at the anomaly and follow-up scans because velamentous insertion, low-lying placenta or cord entanglement alter delivery planning. The visible amniotic cavity also supports discussion of amniotic fluid volume, fetal swallowing and lung fluid dynamics at this gestation.
