Description
At roughly 25 weeks of gestation the fetus lies in a flexed attitude within the amniotic cavity, head bowed toward the chest, arms drawn across the thorax and the lower limbs folded so that one foot rests near the opposite thigh. Surface detail is well advanced for this stage: the external ear has assumed its definitive helical shape and lies close to the skull, the eyelids remain fused shut, and the fingers and toes are fully separated with visible nail beds and plantar creases. The umbilical cord emerges from the anterior abdominal wall at the umbilicus, coils on itself, and inserts near the centre of the discoid placenta, whose chorionic surface shows branching fetal vessels. The translucent amnion and chorion surround the fetus, suspended in amniotic fluid, with the vascularised myometrium and decidua of the uterine wall forming the outer boundary. The mid to late second trimester marks the threshold of extrauterine viability, when pulmonary saccular development and early surfactant production determine neonatal outcome, making this stage central to counselling in threatened preterm birth.
