Description
The fetus lies flexed within the amniotic cavity, head directed toward the lower right of the frame and the trunk curled so that the knees and feet rise toward the placental disc. Surface detail appropriate to the third trimester is visible: rounded subcutaneous fat over the limbs and cheeks, fine scalp hair, a fully modelled auricle with defined helix and lobule, and separated fingers and toes with formed nail beds. The umbilical cord emerges from the anterior abdominal wall and spirals superiorly, its single umbilical vein and paired umbilical arteries discernible through the translucent Wharton's jelly before they fan out over the chorionic plate. The thin amnion is rendered semi-transparent against the vascular uterine wall, with amniotic fluid filling the surrounding space. At roughly 30 weeks the fetus has usually adopted a longitudinal lie, and this depiction is useful for discussing presentation, fetal position and the amount of remaining space within the uterus before term. The view also shows the anatomy central to obstetric surveillance in the third trimester: placental site and cord insertion, cord coiling, and amniotic fluid volume, all of which are assessed sonographically alongside growth biometry and Doppler studies of the umbilical artery. Preterm birth at this gestation, fetal growth restriction, oligohydramnios and cord entanglement are all more readily explained against a realistic image of the intrauterine relationships.
