Description

The fetus lies curled in the amniotic cavity in a flexed attitude, head turned toward the viewer with the eyelids still fused, the auricle of the external ear well formed, and fine hair covering the scalp. The upper limbs are drawn across the chest with fingers separated and nails present, while the lower limbs are folded so the plantar surface and individual toes face the placental disc. The umbilical cord emerges from the anterior abdominal wall, its spiralled vessels visible through the translucent Wharton's jelly, and inserts into the placenta, whose fetal surface displays branching chorionic vessels radiating beneath the amnion. The surrounding uterine wall forms the softly lit chamber that encloses fetus, cord and placenta together. Twenty-three weeks sits close to the threshold of viability, making this stage central to counselling about extremely preterm birth, antenatal corticosteroid timing, and neonatal intensive care planning. The proportions shown here correspond to what is measured at the mid-trimester anomaly scan and on subsequent growth ultrasounds, where biparietal diameter, head and abdominal circumference and femur length are compared against reference charts. Placental position, cord insertion site and the appearance of the chorionic vasculature are assessed at the same examinations, since abnormal insertion or placental location influences delivery planning and the risk of antepartum haemorrhage.
