Description
At approximately 36 weeks of gestation the fetus fills most of the amniotic cavity, shown here in a flexed attitude with the head turned to one side, the arms drawn against the chest, and the hips and knees strongly flexed so that one foot with clearly formed toes and toenails lies close to the face. The uterine wall forms the surrounding vascularised envelope, while the discoid placenta is seen in profile along the posterior aspect, its maternal surface subdivided and its fetal surface giving rise to the umbilical cord. The cord is depicted with its characteristic helical twist, the paired umbilical arteries and the single umbilical vein visible through the covering of Wharton's jelly. Late third-trimester surface features are evident, including subcutaneous fat filling out the limbs and trunk, well-formed scalp hair, a fully modelled auricle, and smooth, opaque skin. Near-term fetal anatomy of this kind underpins routine obstetric assessment: presentation and lie, degree of flexion, and the position of the cord relative to the fetal limbs and neck are all determined at this stage by ultrasound and by abdominal palpation. The relationship between placental site, cord insertion and fetal position is central to discussions of nuchal cord, cord compression, breech or cephalic presentation, and planning for vaginal delivery or caesarean section. By 36 weeks organ maturation, particularly of the lungs, is largely complete, making this the anatomical reference point for late-preterm versus term birth and for counselling on expected fetal growth and amniotic fluid volume.
