Description
Seen from the inferior aspect, the visceral surface of the right hepatic lobe is dominated by a shallow concavity, the renal impression (impressio renalis), created by contact with the superior pole of the right kidney. Medial to this depression, the hepatic hilum (porta hepatis) sits between the fissure for the ligamentum teres and the fossa for the gallbladder, while the quadrate and caudate lobes frame the central groove complex. Posterolaterally, the bare area margins and the vicinity of the inferior vena cava groove orient the viewer to the posterior surface. A tactile landmark. For teaching topographic anatomy, the renal impression matters because it records the normal relationship between liver, right kidney, and diaphragm, a relationship that shifts with hepatomegaly, nephromegaly, and postoperative or traumatic distortion of the right upper quadrant. Surgeons working through a right subcostal incision or laparoscopically around the hepatorenal recess (Morison pouch) use these inferior surface landmarks to correlate peritoneal reflections and potential spaces where blood or ascites collects. Radiologists also reference the hepatic-renal interface when interpreting subhepatic fluid and mass effect on axial CT or ultrasound.
