Description
Seen from the inferior aspect of an adult male liver, the visceral surface of the right lobe is oriented superiorly in the frame while its inferior margin lies closest to the viewer. A shallow colic impression (impressio colica) occupies the anteroinferior portion of the right hepatic lobe, lateral to the fossa for the gallbladder and anterior to the renal impression. Medially, the porta hepatis and fissures for the ligamentum teres and ligamentum venosum define the central visceral topography, while the hepatic flexure of the colon lies inferior to the liver and creates the groove-like marking along the undersurface. Appreciating the colic impression matters when you correlate operative and imaging anatomy around the right upper quadrant. In open or laparoscopic cholecystectomy and right hemicolectomy, the hepatic flexure can be tethered to Glisson’s capsule by peritoneal reflections, so traction on the colon may transmit to the liver and bleed from the capsule. Radiologists also use the pattern of visceral impressions on CT or MRI to confirm liver orientation and to avoid mistaking normal grooves for laceration, subcapsular hematoma, or focal mass effect along the inferior right lobe. A practical landmark.
