Description
Seen from a posterior perspective in an adult male, the posterior visceral surface of the liver is oriented toward the viewer with the esophageal impression (impressio oesophagea) forming a vertical groove near the superior aspect of the left lobe. The indentation lies adjacent to the bare area and the posterior margin of the left hepatic lobe, where the abdominal esophagus descends toward the cardia of the stomach. Medially it approaches the region of the lesser omentum and porta hepatis, while laterally it blends into the smooth diaphragmatic surface reflections. Subtle surface markings around the indentation help distinguish it from the broader gastric impression inferiorly. This is a small landmark. It matters because the esophageal impression is one of the more consistent visceral surface markings used to orient the left lobe during cross sectional anatomy and operative exposure, and it helps explain why left lobe retraction can influence the distal esophagus during upper abdominal surgery. In teaching operative anatomy for anti-reflux procedures or hiatal hernia repair, correlating the abdominal esophagus to its hepatic contact points clarifies the relationship between the esophageal hiatus, the gastroesophageal junction, and the posterior liver surface.
