Description
Inferior inspection of the male liver highlights the visceral surface of the left hepatic lobe, where a shallow esophageal impression (impressio oesophagea) indents the posterior aspect near the fissure for the ligamentum venosum. Medial to this concavity, the lesser omentum attachment along the fissure for the ligamentum teres and the margins of the porta hepatis would be expected as nearby landmarks. Posteriorly, the impression lies in continuity with the gastric region of the left lobe, reflecting the close apposition of distal esophagus and stomach to the liver in the upper abdomen. A subtle marking. Recognizing the esophageal indentation matters when teaching or planning exposure around the esophagogastric junction, since the left lobe often overlies the abdominal esophagus and can limit access during transhiatal esophagectomy or laparoscopic hiatal hernia repair. The relation also explains why hepatomegaly or a prominent left lobe can contribute to technical difficulty with endoscopic retroflexion at the cardia, and why esophageal pathology can sometimes be described in relation to adjacent hepatic surfaces in operative reports. Topographic anatomy becomes practical here. Use this illustration in gross anatomy labs when orienting students to the visceral surface of the liver, in surgical atlases covering upper GI approaches to the hiatus, or in hepatobiliary and foregut CME materials that clarify peritoneal reflections and contact impressions.
