Description

Running longitudinally along the fibrous pericardium, the pericardiophrenic vein accompanies the phrenic nerve and pericardiophrenic artery as they descend anterior to the lung root toward the central tendon of the diaphragm. A lateral sagittal section of the male thorax also brings the heart into profile, with the right atrium and ventricle positioned anterior to the left atrium, and the coronary arteries and cardiac veins coursing over the epicardial surface within the atrioventricular and interventricular grooves. Posteriorly, the thoracic vertebral bodies and rib cage frame the mediastinum, while the trachea lies superior and slightly posterior to the great vessels, and a portion of the right lung occupies the lateral pleural cavity. This relationship matters because the pericardiophrenic vessels sit on the pericardium where surgical and percutaneous access often occurs, and their proximity to the phrenic nerve explains the risk of diaphragmatic palsy during pericardiocentesis or pericardial window creation. The sagittal lateral perspective clarifies how the vein ultimately drains superiorly into the internal thoracic vein or brachiocephalic vein, a pathway that can enlarge in chronic pericardial inflammation or after cardiothoracic surgery and become a source of unexpected bleeding. A small vein, but a reliable landmark.
