Description

Running along the medial distal thigh, the descending genicular vein is isolated in cyan as it courses inferiorly toward the knee, positioned anterior to the adductor magnus tendon and close to the medial femoral condyle. Proximally it drains into the femoral vein within the adductor canal region, where it lies deep to sartorius and adjacent to the saphenous nerve and femoral artery. Distally, small venous tributaries track toward the perigenicular plexus around the medial aspect of the joint. Dark blue vessels depict the surrounding venous network, with caliber increasing proximally toward the femoral vein. Lateral projection makes the depth relationships legible, which is exactly what you need when teaching the layered anatomy of the distal thigh and knee. The descending genicular vessels are a reliable landmark for the medial knee, and their venous component often enters the conversation during medial parapatellar approaches, harvest or elevation of a descending genicular artery flap, and careful dissection in the adductor canal where bleeding can obscure the saphenous nerve. Surgeons also use these vessels to orient around the medial femoral condyle and adjacent genicular branches when working near the periosteum.
