Seen in lateral orbital section, the ciliary ganglion lies just lateral to the optic nerve and close to the orbital apex, suspended between the lateral rectus muscle and the posterior globe within orbital fat. A fine sympathetic root is traced from the internal carotid plexus into the orbit, traveling with the ophthalmic artery and nasociliary nerve before reaching the posterior aspect of the ganglion. Short ciliary nerves leave the ganglion to pierce the sclera around the optic nerve. Small vessels and connective tissue planes around the annulus of Zinn provide orientation. This lateral perspective matters because the sympathetic root represents the postganglionic pathway from the superior cervical ganglion to the dilator pupillae and ocular blood vessels, a tract that fails in Horner syndrome with ipsilateral miosis and mild ptosis. Following the fibers from the internal carotid plexus also clarifies why carotid artery dissection, cavernous sinus pathology, or orbital apex lesions can produce combined autonomic and ophthalmic findings. Small nerve, big consequences. Teaching and clinical teams can use this plate to anchor lectures on autonomic innervation of the eye, the three roots of the ciliary ganglion (sensory, parasympathetic, sympathetic), and the relationship of the ganglion to the nasociliary nerve, ophthalmic artery, and extraocular muscles.
