Description

Oblique within the orbital apex, the nasociliary nerve (a branch of the ophthalmic division of the trigeminal nerve, V1) courses from lateral to medial as it crosses superior to the optic nerve (cranial nerve II) near the optic canal. The globe sits anteriorly in the bony orbit, with the extraocular muscles wrapping it in concentric fashion; portions of rectus muscle bellies and their tendinous sleeves are visible adjacent to the nerve. Surrounding orbital walls from the sphenoid and adjacent bones form the posterior orbit, with accompanying vascular channels tracking alongside the neural bundles. Crossing of the nasociliary nerve over the optic nerve is a key orientation point when teaching orbital neuroanatomy and when planning approaches to the orbital apex. Decompression for traumatic optic neuropathy, endoscopic management of orbital apex syndrome, and surgery for lesions near the optic canal all demand a mental map of where V1 branches sit relative to CN II and the annulus of Zinn. Small anatomy, big consequences.
