Description
Running in the medial orbit, the nasociliary nerve appears as the intermediate branch of the ophthalmic division (V1) as it passes from the superior orbital fissure into the orbital cavity, positioned between the optic nerve posteriorly and the globe anteriorly. From this profile skull section, you would expect to see it crossing toward the medial wall above the optic nerve, then giving off the long ciliary nerves to the posterior aspect of the eyeball and the posterior ethmoidal and anterior ethmoidal nerves toward the ethmoid. The ciliary ganglion sits lateral to the optic nerve, with the sensory root arriving from the nasociliary nerve while short ciliary nerves depart toward the sclera. Extraocular muscles bracket the pathway, with the superior rectus and medial rectus forming key superior and medial relationships in this plane. Clinically, this is the route that explains why injury at the superior orbital fissure can produce corneal anesthesia and loss of the afferent limb of the corneal reflex via the nasociliary long ciliary nerves. Pain patterns also make more sense here: irritation from anterior ethmoidal involvement can refer to the dorsum and tip of the nose (external nasal branch) even when the primary complaint begins as orbital discomfort.
