Running along the superior margin of the lateral rectus muscle, the lacrimal nerve is traced from the posterior orbit toward the anterolateral angle of the bony orbit, where it courses toward the lacrimal gland in the superolateral quadrant. The globe sits centrally within the orbital cavity, with the optic nerve exiting the posterior pole toward the optic canal, while adjacent extraocular muscles frame the nerve’s path within the intraconal and extraconal spaces. Lateral orbital walls and roof are rendered in section, clarifying how the nerve occupies a narrow corridor between periorbita and the muscle cone. Small arterial and venous channels accompany the neural structures. Clinically, this lateral trajectory matters because the lacrimal nerve is a frequent point of confusion when teaching the branches of the ophthalmic division (V1) in relation to the superior orbital fissure and the annulus of Zinn. Its close relationship to the lateral rectus and the lacrimal gland makes it relevant in lateral orbitotomy planning, lacrimal gland biopsy, and decompression procedures, where dissection along the superolateral orbit can produce postoperative hypesthesia over the lateral upper eyelid and reduced reflex tearing.
