Description
Presented in a lateral orbital section, the globe sits anterior within the bony orbit, with the sclera and cornea continuous at the limbus and the optic nerve exiting posteriorly toward the orbital apex. Slender anterior ciliary arteries course forward on the surface of the sclera, running deep to the tendinous insertions of the rectus muscles, most consistently alongside the superior, inferior, and medial rectus, then turning anteriorly toward the anterior segment. The extraocular muscles bracket the globe in standard anatomical position, while accompanying orbital nerves track posteriorly and superiorly in yellow against the white bony walls and adjacent paranasal sinus spaces. This lateral perspective clarifies how the anterior ciliary arteries, as branches associated with the muscular arteries from the ophthalmic circulation, reach the episcleral and conjunctival plexuses and contribute to the major arterial circle of the iris via anterior segment inflow. Surgeons care about that path. Strabismus procedures that involve rectus recession or repeated muscle surgery can disrupt these vessels under the muscle tendon, increasing the risk of anterior segment ischemia, a complication classically associated with multi-muscle operations and higher-risk eyes (older patients, vascular disease, prior surgery).
