Description
Running vertically in the posterior mediastinum, the descending thoracic aorta sits anterior to the thoracic vertebral bodies and just left of the vertebral column in this lateral sagittal section of the male thorax. Superiorly, the aortic arch curves posteriorly over the root of the lung, while the ascending aorta emerges from the left ventricle within the pericardial sac. The fibrous pericardium and mediastinal pleura create a clear anterior boundary, with the trachea and main bronchus seen superiorly and the right lung partially sectioned lateral to the mediastinum. A slender pericardiophrenic artery is traced descending along the lateral pericardium, accompanying the phrenic nerve toward the central tendon of the diaphragm (just inferior to the field). This view clarifies relationships that matter in the operating room and on cross sectional imaging: the aorta’s posterior position relative to the heart and pulmonary trunk, and its proximity to the vertebral column, esophagus, and pleura. Aortic pathology often announces itself here. Descending thoracic aortic aneurysm or traumatic transection near the isthmus can compress adjacent airway or esophagus, and endovascular stent graft planning depends on understanding the arch curvature and descending segment course. The pericardiophrenic artery is also a practical landmark during pericardiocentesis and anterior thoracic procedures because it tracks the phrenic nerve along the pericardium, where injury risks diaphragmatic paresis.
