Description
Viewed from the lateral aspect of an adult male abdomen, the mesocolic region is defined by the peritoneal fold suspending the large intestine from the posterior abdominal wall. The transverse colon and its transverse mesocolon occupy the superior portion, while the descending colon continues inferiorly toward the sigmoid colon, with the mesocolon reflected posteriorly (posterius) toward the retroperitoneum. Peritoneal relationships are emphasized, with the mesenterium forming a broad, fanlike attachment that positions the colonic segments anterolateral to the kidneys and psoas major, and adjacent to the greater omentum and mesogastrium in the upper abdomen. A lateral plane offers a clear sense of depth that axial diagrams often flatten, making it easier to teach which portions of colon are intraperitoneal and mobile versus secondarily retroperitoneal and fixed. That distinction matters in surgery and imaging: lateral-to-medial mobilization of the descending colon and splenic flexure depends on recognizing the peritoneal reflection and staying in the correct avascular plane (Toldt’s fascia) to protect the left ureter and gonadal vessels. Orientation is everything. It also supports understanding of internal hernia pathways and the spread of inflammatory or malignant disease along mesenteric planes and colic vessels.
