Description
Anteriorly on the superior (diaphragmatic) surface of the liver, a shallow cardiac impression is represented as a smooth depression in the left lobe, classically near the medial segment and adjacent to the falciform ligament. Superior to the hepatic convexity lies the central tendon of the diaphragm, which transmits the contour of the pericardium and heart to the hepatic surface. Medially the impression approaches the bare area and the groove for the inferior vena cava, while laterally it fades into the broader diaphragmatic surface. Subtle anatomy, but real. Clinically, the cardiac impression matters because it marks the intimate relationship between the liver, diaphragm, and heart, a relationship that becomes obvious in conditions that change cardiac size or pericardial volume. Right-sided heart failure with hepatic congestion can enlarge the liver and exaggerate diaphragmatic contact, and cardiomegaly or pericardial effusion can deepen or alter this impression over time. For surgeons and imagers, the concept reinforces why superior hepatic contour on CT or ultrasound can reflect extrahepatic pathology and why diaphragmatic motion, phrenic nerve dysfunction, or prior cardiothoracic surgery may change the expected topography. Use this artwork in gross anatomy and surface anatomy teaching to anchor discussion of hepatic lobar anatomy, peritoneal reflections, and the bare area in relation to the diaphragm. It also supports radiology and hepatobiliary surgery materials when explaining superior liver landmarks during subcostal ultrasound windows, cross-sectional correlation, or operative planning near the suprahepatic inferior vena cava.
