Description
Curving along the inferior (visceral) surface of the right hypochondrial liver, the gastric impression (impressio gastrica) appears as a concavity positioned anteromedially, where the stomach contacts the left hepatic lobe. Medial to it, the fissure for the ligamentum teres (fissura ligamenti teretis) and the porta hepatis provide orientation on the same visceral plane, while the gallbladder fossa (fossa vesicae felleae) lies more to the right and anterior. Posteriorly, the adjacent omental bursa region and the lesser curvature relationship are implied by the contour change of the hepatic tissue. Surface anatomy matters here. Teaching the gastric impression clarifies that the liver is not a uniform organ but a molded structure shaped by its neighbors, and it helps explain why the left lobe can appear flattened or grooved in normal anatomy without implying focal pathology. In upper abdominal surgery and endoscopy correlation, this relationship underpins how a distended stomach can alter hepatic surface configuration and why surgeons use the visceral impressions as landmarks when approaching the lesser omentum, the hepatogastric ligament, and the porta hepatis.
