Description
Posterior orientation of the right (dexter) lung is presented with the apex positioned superiorly and the diaphragmatic surface inferiorly, emphasizing the relationship of the superior, middle, and inferior lobes to the posterior thoracic wall. The oblique fissure runs inferoanteriorly from the level of the T3 to T4 spinous processes toward the diaphragm, while the horizontal fissure, when visible posteriorly, meets the oblique fissure near the lateral margin to demarcate the middle lobe. Along the medial border, the hilum region sits anteromedially relative to the posterior surface, where the lobar architecture frames the entry point for the bronchus and pulmonary vessels. Posterior lung anatomy matters because many clinically relevant findings declare themselves from the back first.
