Description
Posteriorly oriented, the secondary (lobar) bronchus is presented with the intermediate bronchus in continuity, emphasizing the broad caliber of the airway trunk before it arborizes into segmental (tertiary) bronchi. From this back-facing perspective, the segmental tributaries diverge laterally and inferiorly toward their respective bronchopulmonary segments, while the more proximal lumen remains medial within the hilum. Branch points appear as discrete posteriorly visible carinae. Short segments, clear geometry. A posterior view matters because the segmental takeoffs you target during bronchoscopy and lung resection are often discussed relative to the posterior membranous wall, where cartilage rings are incomplete and small mucosal lesions can hide in folds. This orientation also supports teaching the right-sided pathway: the intermediate bronchus continues distal to the right upper lobar bronchus and then gives rise to the middle and lower lobar bronchi, a common source of laterality errors in learners and a real problem when correlating endoscopic findings with CT. When planning segmentectomy or interpreting post-obstructive collapse, knowing which segmental bronchus arises from which posterior branch point keeps you honest.
