Description
Viewed from a posterior thoracic perspective, the left main (primary) bronchus courses inferolaterally from the tracheal bifurcation toward the left hilum, with its C shaped hyaline cartilage rings and intervening annular ligaments keeping the airway patent. Superior and slightly lateral, the aortic arch would be expected to sweep over the root of the left lung, while the esophagus lies posterior and slightly medial to the bronchus along the midline. At the hilum, the bronchus sits posterior to the left pulmonary artery in the classic RALS relationship (Right artery Anterior, Left artery Superior). Orientation is clear. This posterior placement matters when you are teaching or planning airway access on the left, because the longer, more horizontal left main bronchus changes the angle of bronchoscopy and the way a double lumen tube seats for one lung ventilation. Its cartilage supported wall resists collapse, but extrinsic compression from mediastinal masses or an enlarged left atrium can still narrow the lumen, a pattern often appreciated on CT and on bronchoscopy as a fixed, posteriorly related indentation. Carinal and main bronchus tumors also track along these rings and can obstruct lobar inflow early.
