Description
Seen in right lateral orientation, the trachea descends into the thorax and bifurcates at the carina into the right and left main bronchi (primary bronchi). The right main bronchus runs more inferiorly and more vertically than the left, passing toward the right hilum anterior to the esophagus and alongside the expected course of the azygos arch. Superior and posterior to the right bronchus, the right pulmonary artery and main bronchial vessels would be anticipated at the root of the lung, with the left main bronchus angling more obliquely toward the left hilum across the midline. Right-sided bronchial anatomy matters because the more in-line trajectory of the right main bronchus makes it the common destination for aspirated material, endobronchial foreign bodies, and misplaced endotracheal tubes that slip into a mainstem bronchus. One-sided ventilation. This lateral viewpoint also supports teaching of bronchoscopy and airway management landmarks, since the carina, the proximal right main bronchus, and the takeoff angle to the left are key reference points when confirming tube position or evaluating central airway obstruction from tumor, mucus plugging, or post-intubation stenosis.
