Description
Arising inferior to the carina, the left main (primary) bronchus is shown from a superior perspective as it courses inferolaterally toward the left lung hilum. The airway’s oblique trajectory is apparent, with the superior lobar (upper) bronchus branching more anteriorly and superiorly relative to the inferior lobar (lower) bronchus as the divisions separate. Medial to the bronchial lumen, the mediastinal side of the root of the lung is implied, while the lateral aspect aligns with the pulmonary parenchymal entry point at the hilum. Appreciating the left main bronchus from above matters because its angle and length differ from the right, influencing both aspiration patterns and airway instrumentation. Left-sided endobronchial intubation can obstruct ventilation to the left upper lobe if the tube tip sits distal to the takeoff of the superior lobar bronchus, a common teaching point during bronchoscopy and one-lung ventilation planning.
