Oriented to the diaphragmatic aspect of the heart, this posterior presentation centers on the inferior (diaphragmatic) surface formed predominantly by the left ventricle with a substantial contribution from the right ventricle. The posterior interventricular sulcus marks the boundary between the ventricles and runs inferiorly toward the apex, while the atrioventricular (coronary) sulcus separates the ventricular mass from the atrial base superiorly. Expect the inferior margin to contour the diaphragm, with the apex positioned anteroinferiorly and to the left relative to the midline. Clean topography. Clear ventricular demarcation. That geography matters because the inferior surface is the standard external correlate for the posterior descending artery course in a right-dominant circulation, and it frames the territory at risk in inferior wall myocardial infarction. Clinically, infarcts in this region often reflect right coronary artery occlusion and may involve the right ventricle, so understanding where the right ventricle contributes to the diaphragmatic surface helps explain associated hypotension and sensitivity to preload reduction. The posterior interventricular sulcus is also a surgical and imaging landmark when correlating coronary angiography, CT coronary anatomy, and echocardiographic views that infer inferior wall motion.
