Description
Right lateral contours frame the inferior (diaphragmatic) surface of the heart, with the apex directed inferiorly and leftward while the base lies superior and posterior. The right ventricle typically occupies the anterior portion of this surface, and the right atrium forms much of the right border, while the left ventricle contributes more posteriorly toward the apex. Between chambers, the coronary (atrioventricular) sulcus and posterior interventricular sulcus define the external planes where the right coronary artery, posterior descending artery (often the posterior interventricular branch), and coronary sinus course. Relationships to the central tendon of the diaphragm and the bare pericardial reflections are implied by the inferior orientation. Clinically, the diaphragmatic surface matters because it is the anatomic substrate for the so called inferior wall territory on ECG and for right coronary artery related infarction patterns. Posterior (inferior) myocardial infarction can involve the AV node region near the crux cordis, so a contour focused on the right sided inferior margin helps correlate surface anatomy with conduction disturbances such as bradycardia or varying degrees of AV block. It also supports teaching of pericardial constraint and why diaphragmatic irritation can refer pain to the shoulder via the phrenic nerve. Cardiology and anatomy faculty can pair this view with coronary dominance diagrams, ECG lead localization (II, III, aVF), and operative descriptions of posterior coronary graft targets. It also fits well in radiology or echo teaching when explaining why subcostal windows interrogate inferior cardiac structures against the diaphragm.
