Description
Posteriorly oriented lungs are rendered in color to emphasize the vertebral surfaces (facies vertebralis) and their relationship to the thoracic cage. Along the medial border of each lung, the paravertebral gutter forms a deep longitudinal impression where the pleura drapes adjacent to the thoracic vertebral bodies. Laterally, this vertebral surface blends into the costal surface (facies costalis) against the ribs, with the posterior portions of the lobes projecting inferiorly toward the diaphragmatic surface. Teaching the vertebral surface from a posterior view helps explain why posterior auscultation is so productive, and why dependent lung regions are examined carefully in the upright and supine patient. Posteriorly, pleural disease often tracks along the paravertebral recess, and small effusions or pleural thickening can blunt the normal contour near the costovertebral angles on radiographs. This relationship also matters during posterior thoracic procedures, where needle paths for thoracentesis or pleural biopsy must respect the proximity of rib heads, intercostal spaces, and pleura near the vertebral column.
