Description
Colored rendering of the left lung is oriented in medial (mediastinal) view with emphasis on the vertebral surface (facies vertebralis) and its broad posterior contour against the thoracic wall. Along the posterior margin, the costovertebral profile transitions anteriorly toward the sharper anterior margin, while the oblique fissure separates the superior and inferior lobes as it runs inferoanteriorly. The lung root region sits centrally, where the mediastinal surface meets the hilum and blends posteriorly into the vertebral impression area adjacent to the thoracic spine. Posterior is dominant. This angle matters when you need to teach or plan around mediastinal relationships that are less obvious on a lateral external view, including how the left lung’s vertebral surface accommodates the paravertebral gutter and how the oblique fissure changes the surgical corridor to a posterior segmentectomy. Correlating the vertebral surface with posterior auscultation and percussion landmarks also helps explain why lower lobe pathology, such as aspiration pneumonia or dependent atelectasis, often projects more posteriorly on physical exam and in supine imaging. The vertebral impression area is a practical reference when discussing pleural reflection and the risk of iatrogenic pneumothorax during posterior thoracic procedures.
