Description
Posteriorly oriented lungs are presented in profile, emphasizing the vertebral surfaces (facies vertebralis) where each pulmonary organ lies immediately lateral to the thoracic vertebral column. Along the medial border, the mediastinal surface (facies mediastinalis) turns anteriorly toward the hilum, while the costal surface curves laterally to meet the posterior ribs. Superiorly, the apex rises above the level of the first rib, and inferiorly the base slopes onto the diaphragm, with the posterior margins tapering toward the costodiaphragmatic recess. Appreciating the vertebral surfaces matters when you are teaching why posterior thoracic findings localize the way they do. Breath sounds and crackles from lower lobe pneumonia often project best over the posteroinferior lung fields, and pleural effusions collect dependently near the posterior costodiaphragmatic recess when a patient is supine. The same posterior relationship underpins common procedural corridors, including posterolateral thoracotomy and video-assisted thoracoscopic surgery ports, where surface anatomy guides entry while protecting the hilum and posterior intercostal neurovascular bundles. Pulmonology and radiology courses often pair this view with PA and lateral chest radiographs to link the vertebral surface to posterior lung segments and to the retrocardiac and paravertebral regions where disease can hide.
