Description
Medial inspection of the right lung’s facies mediastinalis brings the hilum into focus, with the root of the lung centered between the anterior and posterior margins and bracketed superiorly by the apex and inferiorly by the diaphragmatic surface. Anterior to the hilum, the cardiac impression is apparent, while a broad groove for the superior vena cava and the vertical sulcus for the azygos vein track along the anterosuperior and posterior aspects, respectively. The mediastinal pleural surface remains smooth overall, but its concavities map directly to adjacent organs. Orientation is immediate. That pattern of impressions matters when you are teaching laterality and mediastinal relationships, because the right lung’s venous grooves and cardiac impression differ in shape and position from the left. Surgical teams also rely on these landmarks during VATS and open approaches for right upper lobectomy or mediastinal lymph node sampling, where the proximity of the pulmonary hilum to the superior vena cava, azygos arch, and pericardium sets the safe dissection planes. Misreading the mediastinal surface can turn a routine exposure into a vascular complication. Pulmonary anatomy modules and thoracic surgery texts use this view to correlate gross anatomy with CT, where the azygos arch and superior vena cava define right paratracheal and hilar contours, and with pathology specimens in lung resection teaching sets.
