Description
Seen from a posterior perspective, the paired lungs are rendered with emphasis on the costal surface (facies costalis), the broad convex area that lies immediately deep to the ribs and intercostal spaces. The posterior borders of the right and left lungs descend vertically toward their inferior margins, while the apex of each lung projects superiorly toward the thoracic inlet. Laterally, the costal surface wraps around the thoracic cage; medially it approaches the vertebral column where the posterior mediastinal contours begin, even if the mediastinal (facies mediastinalis) details are not the focus. Posterior costal-surface anatomy matters when you are teaching where the lung actually contacts the chest wall and where it does not. Percussion and auscultation of the posterior thorax, scapular retraction for lung examination, and the expected surface projection of lobes all depend on understanding how the lung’s convexity tracks the ribs. Small differences between right and left also have consequences: the left lung’s reduced volume to accommodate the heart alters posterior rib-level landmarks for effusions and consolidation, while the inferior margins relate directly to the pleural recesses where fluid collects and where a thoracentesis is planned to avoid the lung.
