Description
Curving laterally from apex to base, the costal surface (facies costalis) of the lung is rendered as it lies against the inner contour of the thoracic cage. Visceral pleura invests the pulmonary parenchyma and meets the parietal pleura lining the ribs, creating a smooth pleural interface across the costal pleura. Subtle rib imprints and the change in contour toward the inferior border orient the viewer to the lateral thoracic wall and the dome of the diaphragm inferiorly. Clinical work on the pleural space often begins here because the costal pleura is the structure traversed by most thoracentesis and chest tube approaches. The relationship between the ribs and pleura is not academic, misplacement can injure the intercostal neurovascular bundle (VAN) running along the inferior margin of each rib, or violate lung tissue if the pleural reflection and lung edge are misunderstood. Pneumothorax, hemothorax, and pleuritis all declare themselves along this rib facing surface, where pleural sliding and separation become diagnostic targets on ultrasound and CT.
