Description
Costal (rib-facing) facies of the lung is rendered as a smoothly convex lateral surface that conforms to the inner thoracic wall, with the apex positioned superiorly toward the thoracic inlet and the base directed inferiorly toward the diaphragm. Along its anteroinferior margin, the costal surface sweeps into the sharp margo anterior and margo inferior, while the more posterior extent rounds toward the vertebral region and paravertebral gutter. Broad, shallow costal impressions correspond to the overlying ribs and intercostal spaces, creating the characteristic rib interface on the parietal-facing side of the pulmonary organ. Orientation to the costal surface matters whenever you need to distinguish visceral pleural contact from medial hilar anatomy and to teach how the lung molds to the thoracic cage in respiration. Rib impressions become clinically relevant in pleuritic pain syndromes and in the interpretation of peripheral lung collapse, where subpleural atelectasis often tracks along the costal pleura and can mimic a thin peripheral opacity on radiographs or CT lung windows.
