Description
External occipital protuberance (inion) occupies the midline of the squamous part of the occipital bone in a fixed posterior view of the skull base. From this central prominence, the superior nuchal lines extend laterally on both sides, while the external occipital crest descends inferiorly toward the foramen magnum along the midsagittal plane. The surrounding occipital surface transitions from the convex occipital squama superiorly to the more inferior, nuchal region where muscle and ligament attachments concentrate. Midline anatomy dominates. Palpable surface anatomy makes the inion a day-to-day landmark in the clinic and operating room, used to orient posterior scalp incisions and to approximate underlying midline structures at the craniocervical junction. The external occipital protuberance anchors the ligamentum nuchae and provides attachment for trapezius and occipitalis, so hypertrophy, traction, or enthesopathic change at this site can accompany chronic cervicogenic headache patterns, and it sits in the neighborhood of the greater occipital nerve as it courses superiorly to the posterior scalp. Subtle asymmetry can be normal. The isolated posterior perspective keeps attention on bony relief, allowing the nuchal lines and crest to read as true topography rather than being lost in surrounding facial or cranial landmarks.
