Description
Arising from the bronchus intermedius, the right inferior lobar bronchus courses inferolaterally within the hilum toward the lower lobe of the right lung. From a right lateral orientation, its segmental branching into the basal bronchi is emphasized, typically including the medial basal, anterior basal, lateral basal, and posterior basal segmental bronchi, with the superior (apical) segmental bronchus originating more proximally and running superiorly within the lower lobe. The bronchial lumen is supported by cartilage plates, while the accompanying pulmonary artery branches lie superior to the bronchi in the right lung (RALS), and pulmonary veins occupy a more anterior and inferior plane. Bronchopulmonary segmental anatomy is not academic trivia in the right lower lobe. Segment-directed bronchoscopy, endobronchial ultrasound navigation, and selective suctioning for mucus plugging often hinge on recognizing the takeoff angles of the basal segmental bronchi when a patient is positioned in lateral decubitus.
