Description
Arising from the right superior lobar bronchus, the posterior segmental bronchus (B2) courses laterally and then sweeps posteriorly toward the apex of the right upper lobe. From a right lateral orientation, its lumen tracks behind the anterior segmental bronchus (B3) and lies superior to the middle lobe bronchus as it turns toward the posterior aspect of the pulmonary apex. Peripheral branching into smaller segmental and subsegmental bronchi outlines the posterior segment of the upper lobe, with the bronchial wall closely accompanied by pulmonary arterial branches and venous drainage that runs more intersegmentally. Airway anatomy drives segmental boundaries. Segmental bronchial mapping matters when disease respects an anatomic segment, and the posterior segment of the right upper lobe is a classic site for aspiration-related infection in supine patients. A clear lateral view helps distinguish B2 from adjacent upper-lobe segmental bronchi when planning bronchoscopy, segmentectomy, or interpreting lobar and segmental atelectasis patterns on CT. Misidentifying B2 can shift a biopsy, lavage, or resection to the wrong segment. Small errors, big consequences.
