Description
Arising from the right lower lobar bronchus, the right posterior basal segmental bronchus (B10) courses inferoposteriorly toward the diaphragmatic surface, supplying the posterior basal bronchopulmonary segment of the right lower lobe. In standard anatomical position, B10 lies posterior to the more anterior basal segmental bronchi (B7 to B9) and travels with its segmental pulmonary artery branch while pulmonary veins tend to run in the intersegmental planes. Distally, B10 divides into subsegmental bronchi that fan laterally and posteriorly within the basilar parenchyma. Segmental bronchial anatomy matters most when pathology respects segmental boundaries or when an intervention must. Posterior basal disease is a familiar pattern in aspiration pneumonia, where dependent right lower lobe segments, including B10, often collect aspirated material in supine patients, and it is also a common target region for endobronchial sampling in hemoptysis or suspected peripheral malignancy. For thoracic surgeons, the posterior basal segment is a frequent candidate for anatomic segmentectomy, and recognizing the B10 bronchus in relation to the basilar arterial branches and intersegmental veins helps prevent wrong-segment resection and postoperative air leak.
