Description
Arising from the right main bronchus and continuing as the bronchus intermedius, the right middle lobe bronchus gives off a medial segmental bronchus (B5) that courses anteromedially toward the cardiac impression of the lung. Segmental bronchial cartilage plates and a ciliated mucosa overlie a vascular submucosa, while a circumferential smooth muscle layer separates mucosa from the deeper peribronchial connective tissue. Segmental pulmonary arteries typically run superior to their corresponding bronchi, with segmental veins tracking in the intersegmental planes more peripherally. Orientation is everything. Teaching the medial segment of the right middle lobe matters because B5 sits close to the right atrium and pericardium, a relationship that becomes clinically concrete in bronchoscopy, lobar collapse patterns, and CT correlation. Aspiration and mucus plugging often preferentially involve the right-sided bronchial tree given its wider, more vertical main bronchus, and obstruction at the middle lobe bronchus or B5 can contribute to recurrent atelectasis in right middle lobe syndrome.
