Description
Oriented in a rightward lateral presentation, the medial basal segmental bronchus of the right lower lobe (B7) is traced as it leaves the inferior lobar bronchus and courses inferomedially toward the mediastinal surface. Proximally, its origin sits distal to the bronchus intermedius and just beyond the takeoff of the superior segmental bronchus (B6), then it angles anterior and medial relative to the posterior basal (B10) and lateral basal (B9) segmental bronchi. Distally, the airway arborizes toward the parenchyma that abuts the inferior pulmonary vein and the diaphragmatic surface. Orientation matters. Clear delineation of B7 matters because segmental bronchi define surgical and bronchoscopic roadmaps in the right lower lobe, where the basal pyramid can be confusing even for experienced operators when viewed endobronchially. Medial basal segmentectomy and basal subsegmentectomy depend on correct identification of the B7 bronchus in relation to the inferior pulmonary vein and accompanying segmental arteries, and an anteromedial course can place this segment at risk during fissureless approaches or lymph node dissection along the inferior hilum. This is also the segment commonly targeted when correlating bronchopulmonary segments with localized consolidation or aspiration-related disease in the dependent lung base on CT.
