Description
Seen from the right side, the aortic root (radix aortae) rises superiorly from the left ventricular outflow tract as the proximal segment of the ascending aorta. The aortic annulus sits inferiorly as a firm fibrous attachment at the level of the aortic valve cusps, while the sinuses of Valsalva expand immediately above each cusp before narrowing at the sinotubular junction. From this projection, the right coronary sinus lies anterolateral and gives origin to the right coronary artery, whereas the noncoronary sinus is positioned more posteriorly toward the interatrial septal region. Orientation is everything. This rightward perspective matters because it clarifies where the aorta is anchored and where it is not, a distinction that underlies both pathology and repair. Annuloaortic ectasia and bicuspid aortic valve associated aortopathy often dilate the sinuses and proximal ascending aorta while the fibrous annulus remains the key reference for sizing and for planning valve sparing root replacement. During aortic root surgery, the right coronary ostium and the sinotubular junction guide coronary button mobilization and reimplantation, and this projection keeps their spatial relationship to the ascending aorta easy to teach and hard to misread.
