Description
Arising superiorly from the left ventricle, the ascending aorta courses upward in the midline before curving posteriorly and to the left as the aortic arch (arcus aortae), then continuing as the descending thoracic aorta. From an anterior perspective, the brachiocephalic trunk typically emerges most anterior and rightward, with the left common carotid artery ascending more medial, and the left subclavian artery passing superolaterally toward the left upper limb. The arch’s convexity lies superior, while its concavity faces inferiorly toward the pulmonary trunk region and the expected location of the ligamentum arteriosum. Clear orientation. An anterior presentation of the entire aortic arch is a workhorse view for teaching great vessel topology and for correlating cross sectional imaging where branch order and laterality matter. Catheter based interventions often depend on this geometry: negotiating a guidewire from the ascending aorta into the brachiocephalic trunk, left common carotid, or left subclavian requires an accurate mental model of the arch’s curvature and the takeoff angles. This view also supports recognition of common variants such as a bovine arch pattern and helps explain why arch elongation and unfolding in aging can complicate endovascular access and mimic mediastinal widening on frontal chest radiographs.
