Description
Oriented in an anterior (sternocostal) view, the heart is shown with the left atrioventricular (coronary) sulcus carrying the circumflex branch of the left coronary artery as it courses leftward from the left main trunk, deep to the left atrial appendage and along the obtuse margin. Several anterior ventricular branches arise from the circumflex and descend inferiorly across the anterolateral wall of the left ventricle toward the apex, running superficial to the myocardium beneath the visceral pericardium (epicardium). The ascending aorta sits superior and medial, with the pulmonary trunk and proximal pulmonary arteries anterior to it, framing the origin of the coronary circulation. Small-caliber arterioles and paired venous channels track nearby in the interventricular and atrioventricular grooves. Clear landmarks. These anterior ventricular branches matter because their size and distribution vary with coronary dominance and collateral patterns, shaping which regions of the lateral left ventricular myocardium become ischemic in circumflex occlusion. Interventional cardiologists often correlate this surface course with angiographic obtuse marginal branches and with ST-segment changes in lateral wall infarction (I, aVL, V5 to V6), while surgeons respect these vessels when placing sutures near the left atrial appendage or during mitral valve exposure. On cross-sectional imaging, the epicardial fat in the atrioventricular groove can obscure them, so a clean anterior surface rendering helps anchor the expected trajectory.
