Description
Oriented to the left aspect of the heart, the circumflex artery courses within the left atrioventricular (coronary) sulcus as it wraps posteriorly from the left coronary artery, giving off multiple atrial branches that ascend superiorly over the left atrial wall. Several of these small-caliber vessels track toward the left atrial appendage (auricle) and the atrial roof near the pulmonary venous ostia, while more inferior branches remain close to the sulcus at the junction of left atrium and left ventricle. The aorta rises superiorly and anteriorly at the base, with the pulmonary trunk and proximal pulmonary arteries adjacent, providing clear landmarks for the left coronary origin and early epicardial course. Epicardial fat and pericardial surface context help separate atrial branches from ventricular marginal branches on the obtuse (left) border. For teaching coronary anatomy, this left-sided external view clarifies a point clinicians often gloss over: atrial perfusion is not just a right coronary story. Circumflex atrial branches can supply the left atrium and, in a left-dominant circulation, contribute to territories near the atrioventricular groove that matter during mitral valve surgery, left atrial appendage occlusion, and catheter ablation around the pulmonary veins.
