Description
Dominating the anterior surface of the heart, the circumflex artery (ramus circumflexus of the left coronary artery) tracks leftward in the coronary (atrioventricular) sulcus, curving around the obtuse margin toward the posterior aspect. Small atrial branches arise from its proximal segment and course superiorly over the left atrium and left auricle, running just inferior to the pulmonary trunk and left atrial appendage. The ascending aorta sits superior and central at the base, while the left ventricle forms most of the anterior and left lateral myocardium, tapering to an inferolateral apex. Superficial coronary veins accompany the arterial pattern in parallel grooves on the epicardial surface. Small vessels matter here. These atrial branches define the left atrial blood supply and their relationship to the left atrial appendage, a common site of thrombus in atrial fibrillation and a frequent target for occlusion devices. During mitral valve surgery or left atrial ablation, knowing where the circumflex artery hugs the atrioventricular groove helps avoid iatrogenic injury, and atrial branch compromise can contribute to atrial ischemia and postoperative arrhythmias. Coronary dominance also enters the picture because a dominant circumflex may contribute more extensively to the posterior left ventricle and atrioventricular nodal region.
