Description
Oblique left anterolateral orientation brings the left atrium and left ventricle into the foreground, with the left atrial appendage draped anterior to the atrioventricular (coronary) sulcus. The circumflex branch of the left coronary artery tracks leftward in the coronary sulcus beneath the atrial appendage, then gives off anterior ventricular (left ventricular) branches that descend on the obtuse (left) margin toward the anterolateral left ventricular myocardium. Superiorly, the ascending aorta and aortic arch rise from the left ventricular outflow tract, while epicardial fat and superficial pericardial reflections partially cushion the coronary vessels within the sulci. Small-caliber arterioles are suggested as distal arborization over the ventricular surface. Clean surface anatomy. These anterior ventricular branches from the circumflex artery define a common pattern of perfusion for the lateral wall of the left ventricle, the same territory implicated in lateral STEMI when a dominant circumflex supplies broad myocardium or when obtuse marginal branches occlude. For surgeons and interventionalists, their relationship to the left atrial appendage and the coronary sulcus matters during mitral valve surgery, left atrial appendage exclusion, and when selecting targets for coronary artery bypass grafting to obtuse marginal or diagonal-equivalent vessels. The angled view also helps teach why circumflex lesions can hide on a standard 12-lead ECG. Lateral wall ischemia is easy to miss.
